How to Apply - The Referral Process
To begin counselling and/or use our other services you need to apply to join the Centre for Better Health which requires completing an assessment form and obtaining a referral.
Source of Referrals
There are several ways you can be referred to C4BH:
- By self-referral
- By your doctor / GP
- By the social services
- By a voluntary agency
- By hospital
- By a housing association
The Assessment Form
Please complete our assessment form or ask someone to complete it on your behalf. There are several ways to obtain the form:
- Daytime Counselling (PDF, 124Kb)
- Evening Counselling (PDF, 67Kb)
- Stress Management (PDF, 7Kb)
- Training Courses (PDF, 133Kb)
- By request - phone, email or write to us with your name and address and we will send you a form in the post.
- Collection - visit our Groupwork centre and pick up a form yourself.

What happens Next?
If applying for counselling:
Upon receipt of a completed assessment form we will match your availability and suitability with one of our counsellors. After your first session you should have a clear idea of what you expect from counselling.
If applying for other services:
Upon receipt of your completed application form we will arrange a time with the centre manager for you to visit, and see the facilities. Following this you will be eligible to become a member of C4BH.
